Apply to Ocala Adventist Academy

In order to become eligible to enter Ocala Adventist Academy, each prospective student and his/hear parent/guardian must do the following (applicants may be placed on a waiting list until an opening becomes available):

  1. Download the Prospective Student Application Form below and submit the completed form with $50 nonrefundable Application Fee:

  2. Download the Prospective Student Admissions Form below and submit the completed for with nonrefundable Registration Fee:

    • Call (352) 509-5043 to schedule a drop-off
  3. Schedule student assessment, unless student has taken the Stanford, Terra-Nova, or MAPS test within the past year and can provide a copy of testing results.
  4. Submit all necessary documentation:
    • Copy of Certified Birth Certificate
    • Florida Immunization Form
    • Copy of Parent/Guardian ID
    • Acceptance Letter (Step Up Students)
    • Copy of most recent Report Card
    • Copy of transcript (for high school students)
    • Copy of most recent achievement test scores
